Specialized Medical Supplies (SMS) are Medicaid-covered items authorized under DBHDD waiver services to support individuals’ health, safety, and functional independence in home and community-based settings. These supplies are medically necessary and are intended to assist with the management of identified conditions, prevent complications, and promote optimal health outcomes.

All Specialized Medical Supplies must be prescribed or ordered by an appropriate licensed practitioner and must be included in the individual’s Person-Centered Plan (PCP) or Individual Service Plan (ISP) when required. Authorization is based on documented medical necessity and in accordance with Medicaid and DBHDD guidelines.

Covered supplies may include items such as wound care products, incontinence supplies, adaptive equipment, nutritional supplements (when medically necessary), and other health-related items that support daily functioning and reduce the risk of institutionalization. Services are limited to items that are not otherwise covered by the Medicaid State Plan or other funding sources.

The provision of Specialized Medical Supplies is coordinated to ensure timely access, proper utilization, and compliance with all applicable Medicaid and DBHDD policies. All supplies must be used as prescribed and documented appropriately to support ongoing care planning and service delivery.
